Please fill out the FollowMyHealth Patient Portal Registration Form to request access to FollowMyHealth for direct access to your electronic medical records and communicate with your provider. Someone will then contact you to assist in granting access. For patients who have established access to the patient portal, you can gain direct access to your electronic medical records and communicate with your provider here.
